Pet Health Club - How to Join

To join the Pet Health Club either give us a call on 01444 456886 or download an registration form here. Bring it in or send it to us and we'll do the rest.

Alternatively come into the Clinic and we will complete the registration form with you. This will require details of your bank account in order to set up a direct debit; so please bring the details with you. Our staff will be happy to deal with any queries you may have.
